Appointment Information


Developmental Behavioral Pediatrics LLC schedules visits by appointments only. To set up an appointment, please call us at (973) 437-6525. Please note once a new appointment has been scheduled you will be charged a Missed Appointment fee of $100.00 for any missed appointments or late cancellations (less than 24 hours notice.)


How to Prepare For Your Appointment 


New Patients:


Call our office 973-437-6525. Parents will be asked to complete screening forms and submit them electronically via our website. If you have additional medical or school records, early childhood photos etc. that will assist in the evaluation process you may submit them via fax, email or bring a copy of the documents with you to the office visit. Please call our office within in one business day of submitting your forms to confirm that DBPEDSLLC has received your forms.


Once we receive your forms and they have been reviewed you will can call to schedule an appointment and we will provide you with additional information.


Please arrive 20 minutes prior to your appointment time to allow us time to enter your patient information into our computer system and create your chart.


Established Patients:


For each office visit, we ask that you arrive at least 15 minutes early to verify your patient information. 


For Every Established Patient Office Visit, Please:


. Bring a list of all of medications, supplements and vitamins your child is currently taking*
. Review ahead of time what medications you need refilled 
. Advise us of any change in your telephone numbers, home address, or workplace
. Advise us of any changes in your child's medical condition and/or behavior since your last visit with us.


Consultation Fees


Any fees discussed serve as an estimate for assessment and consultative services and will vary depending on the complexity and level of services provided. We have transitioned to a self-pay model and therefore are we are not participating in any Health Insurance plans at this time.  We require full payment in the forms of Debit or Credit Card for all services provided on the same day of service. Some Insurers will offer partial reimbursement for Out-of-Network Services and we will provide you with a billing invoice containing the appropriate medical billing codes describing services provided for you to submit to your insurance company. It is your responsibility to check with your provider prior to making an appointment with our office.


Cancellation Policy


Please understand that you are financially responsible for confirmed appointments and will be charged $100 if you cancel a scheduled visit without adequate notice. New patient evaluations, follow up and parent feedback visits must be cancelled no later than 1 business day (24 hours, or Friday morning if your child’s appointment is on a Monday) prior to the visit. Understand that we will schedule hours of our teams time to be spent with you and your child for a Patient’s Evaluation that cannot be rescheduled without adequate notice.
